The VA needs 21 days notice before a student begins a rotation.
Students must be assigned to a VA rotation within 120 days of having been cleared by the VA.
The Departments of Surgery and Medicine, and some of the Surgery Subspecialties make assignments to the VA.
When you are notified that you have been assigned a rotation at the VA, read the PIV ID Proofing Criteria, complete the General Information Form, e-mail it to Cheryl Adams cheryl.adams@va.gov; then make an appointment with her by phone 424-6135. You MUST complete ALL blocks on the Word document; the PIV criteria tells you what you must bring with you to get your ID badge and to be fingerprinted.
