Q. Who do I contact about the heating or cooling comfort in my office or area?

A. Physical Plant 5-6319.

 

Q. Who do I contact to report a problem that needs immediate attention?

A. Maintenance Control 5-6319.

 

Q. Who do I contact if my building access card does not work?

A. Auxiliary Services at 5-7650 or University Police Department 5-6165.

 

Q. Who do I contact for elevator maintenance problems?

A. UPD at 5-6165.

 

Q. Who do I contact when my telephone is not working?

A. Telecommunications at 5-6505.

 

Q. Who do I contact about call buttons, pneumatic tube system, or television?

A. Biomedical Engineering at 5-6322.

 

Q. Do you setup tables for conference?

A. No, contact the Plant Store at 5-5140; they require an IT (Internal Transaction) for these services.

 

Q. What documentation is needed to get equipment/assets moved or sent to the Asset Management Surplus Warehouse?

A. If equipment/assets are either tagged or non-tagged you must submit a “Notice of Change in Movable Equipment” form (S/N 1333 available only from the Print Shop) to Asset Management/Equipment Control who will then submit a work request to the Physical Plant department to set up the move.

 

Q. Do you have any fans or extension cords to borrow?

A. No. These items should be purchased by individual departments. Extension cords can be distributed only during outages and should be returned promptly.

 

Q. Who do I contact regarding pest control problems, and how are these complaints handled?

A. Physical Plant at 5-6319. The pest control company is on campus every Tuesday for spraying routine areas, plus any problem call-backs. If there is a problem that can't wait until Tuesday, we will call them out on an emergency basis.

 

Q. How do I obtain floor plans of campus buildings?

A. Many floorplans are available on the FACILITIES section of this website. A free utility from AutoDesk is required to view the files. The necessary viewer may be downloaded by clicking here.

 

WORK ORDERS:

Q. How do I enter a work order electronically?

A. Please go to physical plant's website www.sh.lsuhsc.edu/physplant or go to LSUHSC-Shreveport home page and look under 'Hospital Administration' or 'Other Campus Services' for link to Physical Plant. You will find a link to 'Submit a Work Order'. Please have your work location and the account number available before you submit a work order.

 

Q. Who do I contact to check on the status of my work order?

KEY CONTROL:

Q. Where do I pick up a door key?

A. Key Control, Room 235 of the Physical Plant Building. An approved key request form should be submitted to Physical Plant.

 

Q. Who can approve a Building Key Request?

A. All required signatures must be obtained for the key request to be processed. Required approvals are as follows.

  • Hospital – Department Head & Hospital Administration
  • Medical School – Department Head & If they cross over to other departments then Medical School Administration needs to sign
  • Medical School Graduate Students – Department Head & Dr. Sandra Roerig
  • Animal Resources – Department Head & Dr. Hugh Price
  • Allied Health / Mollie Webb – Department Head & Dr. Joseph McCulloch
  • Cancer Center – Department Head & Becky Dekay
  • Clinical Trials – Department Head & David Bloomer
  • Ambulatory Care / WCC – Department Head & Ruth Hawkins or Hosp. Administration
  • Eye Clinic – Debbie Wyman & Ruth Hawkins or Hosp. Administration

 

Q. How do I find out if my key is ready for pick up?

A. Key Control at 5-6352.

 

Q. How do I obtain keys to a file cabinet or desk?

A. Key Control at 5-6352. No key request is needed.

 

Q. How long do I have to pick up my keys once I receive a call that they are ready?

A. Ten working days.

 

Q. Can someone else pick up my keys for me when they are ready?

A. No.

 

Q. How many keys can I get to my door?

A. One.

 

Q. If I lose my keys, can I get a new set?

A. You can go to the cashier cage, room # 1-218 and pay $25.00 per key. Bring the receipt to Key Control and new keys will be issued.

 

Q. If I need to get into my office and I left my keys at home, who do I call?

A. UPD at 5-6165.

 

Q. If my key won’t work in the lock or it breaks, who do I call?

A. Physical Plant at 5-6319.

 

Q. Where do I go to get my card programmed for a room?

A. Room 223 of Physical Plant Building. Please call Dennis Murray at 5-6255 before coming over.

 

Q. Am I responsible for turning in my keys when I resign?

A. Yes. If you do not turn in your keys, you will be charged $25.00 a key. We can not sign your clearance form until this charge is paid in full or keys are returned.
