 | All patients are required to bring their insurance card to every
clinic visit. |
 | Insurance co-pays are to be collected at the time of registration. |
 | It is the parent's responsibility to obtain the insurance
authorizations or Medicaid referrals from their primary care
physician. |
 | Individual clinics will provide information about where
required Authorization Referral Forms should be sent. These forms
must be received at least one (1) week prior to the patient's scheduled
appointment. |

What is The
LSUHSC-Shreveport Children’s Hospital?
|
 | Our Children's Hospital is affiliated with St. Jude's Hospital in
Memphis, Tennessee. This affiliation brings pediatric oncology
services to children in the Ark-La-Tex area. |
|
|
The LSUHSC-Shreveport Children's Hospital is a primary and
tertiary acute care facility dedicated to the health and well-being of
children in the Ark-La-Tex area. Established in 1996, the Children's
Hospital was designated a "hospital within a hospital".
Physically located within the University Hospital, the
Children's Hospital serves as the pediatric teaching hospital of Louisiana
State University Health Sciences Center School of Medicine in
Shreveport.
Plans are underway to expand our services by building a
"free-standing" children's hospital. To support this

| The
LSUHSC-Shreveport Children's Hospital is staffed by physicians with
special training and skills in the management of complicated acute and
chronic pediatric illnesses. Over 60 full-time faculty members, plus 30
resident doctors care for our pediatric patients.

| Our
inpatient hospital services consist of a 33 bed Infant and Children's
Ward, a newly renovated 8 bed Pediatric Intensive Care Unit, a 40 bed
Neonatal Intensive Care Unit and a Newborn Nursery.
The Women's and Children's Clinic, located on the corner of Kings
Highway and Linwood Avenue, provides convenient access for pediatric
outpatients. There are multiple Subspecialty clinics and programs
offered.
The Children's Hospital also has access to a Positron Emission
Tomography Center, a Level One Pediatric Trauma Center, and a Regional
Burn Center. |

Our
inpatient hospital services consist of a newly renovated 33 bed infant and
children's ward, staffed by 2 full-time Board Certified Pediatricians, as
well as many other General and Subspecialty Pediatricians.
"Hospitalists" are physicians spending at least 25% of
their time providing care for hospitalized patients referred by their
primary care physicians (PCP). The patients are then referred back to
their primary care physician at the time of discharge.
Advantages of our Hospitalist service:
 | Efficiency: We are familiar with the services in the hospital
and how to efficiently use them. This means fewer days in the
hospital. |
 | Availability: Since we don't have clinic patients and
residents are present 24 hours a day, patients are seen frequently
throughout the day and night. |
 | Continuity of Care: Follow-up phone calls are made to the PCP
during the hospitalization and at the time of discharge. A typed
discharge summary is sent to the PCP a few days later. |
