EVALUATION FORMS

Clinical Evaluation Forms (These are on My Evaluations.) One will be completed for each of the following

	Nursery Rotation: The form in My Evaluations is Pass/Fail with optional comments to be included in your Dean's Letter.
	Ward Rotation: One form completed by your attending and one form completed by your resident
	Clinic Rotation: The form in My Evaluations is Pass/Fail with mandatory comments to improve your performance. One form completed for each 1/2 day you are in the clinic.
