Instructions:

The forms are available in PDF format and they can be filled and printed. Please use the buttons on the form for printing and e-mailing. Member Registration can be done by e-mail since there is no registration fee.

You would need to download Adobe Reader to view/print/e-mail the form. Please download if you already don't have it on your computer.

Member Registration Email Instructions:

Please click on 'Submit by Email', choose your favorite e-mail application when screen prompts,

1. For Desktop e-mail application such as Outlook or Eudora: Select 'Desktop E-mail Application' and Choose 'Send Data File'. A data file with extension '.xml' will be attached to your e-mail, with a Subject. Continue and 'Send' the e-mail.

2. For other internet e-mail services such as Hotmail or Yahoo: Select 'Internet E-mail' and Choose 'Save Data File'. Please read the prompted instructions. Save the data file with extension '.xml' on your computer . Attach the '.xml' file to your e-mail, with a Subject and Send the e-mail.
