As a newly hired employee, you are responsible for completing all employment paperwork and returning it to Human Resource Management (HRM) prior to your start date. Your letter of offer will include your appointment type. Please complete all forms listed under your appointment type, unless you have received notification from HRM that certain form(s) are not required as a condition of your appointment.

Please be sure to print each page on a separate sheet of paper. Sign and date all forms that require a signature. Once all forms have been completed, mail them to:
