Use the following
links to download the IRB documents according to the following method:
- Right-click on the link and
choose Save link (Target) As... to save
the file with a name of your choosing. You may then open the file
in Word 97 (or later version). Be sure to enable the macros if
you are prompted to do so.
Do not save a blank document for later
reuse, as these documents are frequently updated. Always come back
to this site to download as needed so that you will always have the
latest version.
