Insurance Information

	All patients are required to bring their insurance card to every clinic visit.
	Insurance co-pays are to be collected at the time of registration.
	It is the parent's responsibility to obtain the insurance authorizations or Medicaid referrals from their primary care physician.
	Individual clinics will provide information about where required Authorization Referral Forms should be sent. These forms must be received at least one (1) week prior to the patient's scheduled appointment.
