What is an Adjusted Service Date?
The
term "Adjusted Service Date" is used by Civil Service to

2.
Can my Adjusted Service Date and my Retirement Date be different?
How?
Yes.
Remember that your retirement is funded by you. If you ever leave
the state system and take your retirement money with you, the time
you paid into the system would not count toward retirement or retirement
date. If you subsequently came back into the state system, you would
start from zero, just like a new employee. On the other hand, the
time worked could count toward your adjusted service date...making
the two dates different.
3. What
do I need to do to change my name or address?
A name change requires a Social Security card with the name you are
changing to on the Social
Security card. The receipt showing that you have ordered a new Social
Security card can not be used to change your name. Bring the new card
with you to Room 116 of the old Allied Health Building, and complete
a Personal Data Change form.
This
form can also be used for an address change. You can find this form
on this website. Click here for a Personal
Data Change form..
