There are two parts to the PM-11 Form A. Please explain:

The 1st part of the form located in front of the cardboard must be completed by the employee who has outside employment.

The 2nd part of the form located behind the cardboard is divided into three sections: Administrative Review, Administrative Approvals, and Action by Vice Chancellor for Academic Affairs or Shreveport Affairs.

  • Administrative Review: must be completed by Dept. Chair/Head/Director
  • .
  • Administrative Approvals: must be signed/dated by Dept. Chair/Head/Director and checked off as Recommended or Not Recommended.
